Job Duties Conduct thorough history and physical exams for patients in an outpatient setting, applying critical thinking to assess, diagnose, and manage acute and chronic conditions Develop and manage individualized treatment plans, including prescribing medications and recommending therapies based on current clinical guidelines and patient-specific factors. Order and interpret diagnostic tests as needed, integrating results into clinical decision-making and care plans. Deliver compassionate, patient- and family-centered care by providing education on health maintenance, disease prevention, and chronic disease management. Coordinate and manage follow-up care, respond to patient concerns between visits, and facilitate timely referrals to specialists or community resources to support comprehensive care. Ensure accurate, timely documentation of patient visits, care plans, and clinical outcomes in the electronic medical record (EMR). Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure continuity and quality of care across the patient’s healthcare journey. Participate in quality improvement initiatives and contribute to clinic protocols that enhance patient safety, satisfaction, and health outcomes. Promote wellness and disease prevention strategies tailored to the needs and circumstances of each patient across the lifespan. Maintain ongoing professional development by engaging in continuing education and staying current with evidence-based practice and clinical advancements. Qualifications Education: Master's degree from an accredited Physician Assistant program Current license to practice as a Physician Assistant (PA) by the New York State of New York Basic Life Support (BLS) Certification, required within 90 days of service Experience: 1-2 years experience in outpatient setting, preferred 1-2 years experience in outpatient setting, preferred Comprehensive knowledge of medical terminology, clinical procedures, and patient care techniques Strong clinical skills in assessing, diagnosing, and treating a wide range of medical conditions Proficiency in Electronic Medical Records (EMR) and other healthcare technology Critical thinking, time management, strong communication, problem-solving, and ability to handle various clinical situations.
