Offsite Medical Assistant I Ambulatory-MSW-FT Days

Mount Sinai - - Responsibilities: Assist the provider during patient visits by obtaining vital signs and documenting results in the EMR; Obtain prior authorizations for prescriptions and durable medical equipment; Draft prescription requests in EMR and route for provider review; Perform phlebotomy and collect/process specimens; Perform POCT and document results in EMR; Conduct daily quality control for POCT equipment; Order clinical supplies and stock exam/consult rooms; Assist providers with procedures and act as chaperone as needed; Perform EKGs when ordered; Perform pre-visit planning for diabetic and CHF patients per guidelines; Monitor refrigerator temperatures and biomed expirations for equipment; Participate in daily huddles to review patient lists and treatment plans; Perform age-specific screenings (ADLs, Falls, alcohol, tobacco, depression) annually; Enter medication adherence/diabetes control/foot-eye exam data into EMR; Distribute educational materials to patients; Assist Population Health Management with preventive screenings; Retrieve patient information from outside facilities for transition of care coordination; Assist with follow-up related to Coumadin therapy; Assist at front desk as needed (phone calls, patient registration, discharge, scheduling); Follow Primary Care Medical Home and MACRA protocols
