Our approach supports the whole person while helping families navigate the emotional and practical challenges of caring for a loved one. We are seeking an experienced, compassionate, and proactive Registered Nurse to serve as RN Administrator & Director Of Care throughout Greater Atlanta, GA. This is an opportunity for a nurse who wants meaningful involvement with clients and families, the authority to strengthen care delivery, and the opportunity to help build a growing organization.
Position Summary
The RN Administrator & Director Of Care provides day-to-day agency leadership, nursing oversight within the agency's authorized scope, and hands-on geriatric care management. The role's primary emphasis is client well-being, individualized care planning, caregiver support, family communication, and quality assurance. Community partnerships and responsible business development are additional leadership responsibilities. As the designated Administrator, this individual is accountable to the agency's governing body and has the authority and responsibility to oversee agency operations. The role works closely with operations, scheduling, recruiting, field supervisors, caregivers, and outside healthcare professionals to ensure that care is appropriate, coordinated, and responsive. This position includes regular client visits, assessments, family meetings, staff coaching, and follow-through on care concerns.
Key Responsibilities
Client Care & Nursing Leadership
- Maintain an active presence in clients' homes through assessments, supervisory visits, care conferences, and follow-up visits.
- Assess client needs within RN scope, including functional abilities, cognitive concerns, home safety, caregiver support, and changes in condition.
- Develop and oversee individualized service plans in collaboration with clients, families, caregivers, and relevant healthcare professionals.
- Ensure care recommendations reflect client preferences, safety needs, available resources, and the agency's service capabilities.
- Respond promptly to reported changes in condition and coordinate appropriate communication with treating clinicians, families, and the operations team.
- Provide nursing services only when authorized by the agency's license, policies, applicable orders, and professional scope.
- Guide caregivers in recognizing and reporting concerns and following each client's established plan.
- Promote dignity, independence, informed choice, and respectful treatment of clients and their homes.
Geriatric Care Management & Family Advocacy
- Serve as a consistent resource for families navigating aging, dementia, chronic illness, caregiver stress, and changing support needs.
- Conduct care management consultations and coordinate practical plans for aging in place.
- Help families understand care options and connect with appropriate healthcare and community resources.
- Coordinate with physicians, home health agencies, hospice teams, rehabilitation providers, and other professionals involved in the client's care.
- Support hospital and rehabilitation transitions through timely communication, preparation for home care, and follow-up.
- Identify gaps in support and help resolve barriers to safe, consistent care.
- Facilitate family meetings and communicate recommendations, progress, concerns, and next steps.
- Provide wellness education and coaching appropriate to professional qualifications and client needs.
Agency Administration & Operational Leadership
- Lead daily operations in partnership with agency ownership and the operations team.
- Implement agency policies and maintain readiness for licensing reviews, surveys, and inspections.
- Oversee service delivery, staffing coordination, and timely resolution of operational concerns.
- Collaborate with scheduling and recruiting to match caregiver skills and availability with client needs.
- Evaluate whether prospective clients can be served safely and consistently within agency capabilities.
- Establish clear responsibilities, communication practices, and accountability across the care team.
- Participate in budgeting, resource planning, and service improvement.
- Maintain appropriate leadership coverage during absences.
- Participate in an established after-hours leadership and clinical escalation rotation, with schedule and coverage expectations defined in advance.

- Partner with recruiting and operations to evaluate candidates' readiness for assigned responsibilities.
- Support orientation, competency assessment, ongoing education, and performance coaching.
- Provide practical instruction on dementia support, infection prevention, communication, home safety, and changes in condition.
- Help caregivers understand each client's needs, preferences, routines, and care plan.
- Promote prompt communication with operations and appropriate escalation of concerns.
- Foster teamwork, caregiver recognition, professional growth, and job satisfaction.
- Address performance concerns constructively and follow through on improvement plans.
Quality Assurance & Documentation
- Lead quality improvement efforts focused on client experience, care consistency, and timely follow-through.
- Review client records, service plans, visit documentation, and staff qualifications for completeness and accuracy.
- Ensure assessments, supervisory visits, and service-plan reviews occur within applicable requirements.
- Oversee complaint resolution, incident review, and corrective actions.
- Monitor recurring concerns and implement practical improvements.
- Protect client confidentiality and support appropriate information sharing.
- Ensure client, family, caregiver, and healthcare-provider communications are documented promptly in the agency's designated systems.
Community Partnerships & Responsible Growth
- Build relationships with hospitals, discharge planners, rehabilitation centers, physicians, senior living communities, elder law attorneys, and other referral partners.
- Serve as a knowledgeable nursing resource for families and professionals seeking home care and geriatric care management support.
- Deliver educational presentations on dementia care, transitions home, caregiver wellness, and aging in place.
- Collaborate with business development staff to follow up on inquiries and coordinate appropriate new client starts.
- Identify opportunities to expand services while maintaining staffing capacity and care quality.
- Track partnership activity and contribute to sustainable growth.
Clinical judgment, client choice, and appropriate care recommendations must remain independent of sales targets or compensation incentives.
