Home Health Speech Therapist / Language Pathology (ST) – Santa Clara County: San Jose* Please note: All opportunities at 21HHS require field visitations to patients in their homes. Responsibilities In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.
