Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of clients' psychosocial needs, identifying emotional challenges stemming from serious illness or end-of-life issues.

• Participate actively in the development and implementation of individualized care plans, collaborating with healthcare professionals to ensure holistic support for patients and their families.

• Monitor client progress, adjusting care strategies as necessary based on outcomes and ongoing assessments.

• Facilitate family meetings to provide emotional support and to communicate care plans and resources effectively.

Patient Population / Client Focus\n

This role primarily focuses on senior patients receiving hospice care, many of whom are facing terminal illnesses and require compassionate, end-of-life support. As a clinician in this sensitive environment, you will address a variety of psychosocial aspects affecting these patients and their families, promoting dignity, comfort, and emotional well-being during challenging times.

Qualifications\n

• A Master's degree in Social Work (MSW) or related field from an accredited institution is a required qualification.

• Current Licensure as a Clinical Social Worker (LCSW) in the state of Kansas is necessary to practice.
