Home Health Physical Therapist

Location: Fairfax, Virginia & Northern Virginia Region Job Type: Full-Time / Part-Time

About the Role

We are currently seeking a compassionate, dependable, and motivated Physical Therapist to join a growing Home Health team serving patients throughout Northern Virginia.

This role offers the opportunity to provide one-on-one skilled therapy services in patients' homes, helping individuals regain mobility, improve independence, and safely recover in a familiar environment.

Service Areas

Patient visits are conducted throughout the Northern Virginia region, primarily including:

• Fairfax County
• Falls Church
• Annandale
• Fredericksburg
• Spotsylvania
• Stafford

Key Responsibilities

As a Home Health Physical Therapist, you will:

• Perform initial evaluations, reassessments, follow-up visits, and discharge visits in patients' homes
• Develop and implement individualized treatment plans of care
• Provide skilled interventions including therapeutic exercise, gait training, balance training, functional mobility training
• Educate patients and caregivers on home exercise programs, fall prevention, safety strategies, mobility techniques
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with physicians, nurses, case managers, and interdisciplinary team members
• Complete timely and accurate documentation in the Electronic Health Record (EHR)
• Participate in care coordination and interdisciplinary meetings as needed
• Ensure compliance with Medicare, Medicaid, and agency regulations
• Maintain high standards of clinical care, professionalism, and ethical practice

Qualifications

• Active Virginia Physical Therapist License
• CPR Certification
• TB Screening
• Reliable transportation for patient home visits
• Strong assessment, treatment planning, and clinical decision-making skills
• Ability to work independently in the field
• Knowledge of Electronic Health Records (EHR)
• Familiarity with Medicare guidelines and regulatory compliance

Preferred Qualifications

• Minimum 1 year of clinical experience preferred
• Prior experience in Home Health, Rehabilitation, Outpatient Therapy
• Experience with OASIS documentation strongly preferred
• Strong new graduates with relevant clinical rotations are encouraged to apply
