Your next nursing opportunity awaits as a skilled Registered Nurse with a focus on home health care. In this fulfilling, patient-centered contract role, youll leverage your clinical expertise and exceptional documentation abilities to make a meaningful impact in the lives of homebound clients. This is a 13-week full-time assignment, offering a structured Monday through Friday schedule, with the potential for some on-call responsibilities.

Schedule:

• 13-week contract, full-time
• Monday through Friday
• Daily visit quota; occasional on-call may be required

Responsibilities:

• Deliver high-quality in-home nursing care and skilled interventions
• Complete patient assessments and timely OASIS documentation
• Collaborate closely with physicians and multidisciplinary care teams to create and deliver individualized care plans
• Provide wound care, medication management, patient education, and disease process monitoring
• Communicate effectively with team members regarding patient condition changes and updates

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• OASIS documentation experience required
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Able to maintain a structured full-time schedule and manage multiple daily in-home visits
