Job Opportunity: Home Health Physical Therapist
Our organization is looking for a dedicated and empathetic Home Health Physical Therapist to provide exceptional rehabilitative care in patients' homes. This role is primarily field-based and involves a variety of responsibilities aimed at enhancing patient well-being.
Key Responsibilities- Conduct thorough assessments of patients' physical conditions.
- Develop personalized treatment plans tailored to individual needs.
- Assist patients in regaining mobility and achieving independence.
- Work collaboratively with a multidisciplinary team focused on patient safety and quality care.
- Engage in continuous improvement practices to enhance service delivery.
- Valid physical therapy license.
