LTSS Service Coordinator - Clinician

Job Overview LTSS Service Coordinator-Clinician (Bilingual Spanish/English). The role is field‑based, requiring travel up to 2 hours daily, with occasional office attendance. The position reports under an RN or Licensed Social Worker and is responsible for managing member cases in accordance with state law and contract requirements. Location & Schedule Must reside in the state of Florida and be open to travel up to 2 hours away daily. Monday–Friday, 8a.m.–5p.m. EST. Key Responsibilities Assist responsible RN or LSW in identifying members for high‑risk complications, alternative levels of care, or waiver programs. Obtain clinical data as directed and provide it to RN or LSW for assessment, care plan development, monitoring, and evaluation. Participate in coordinating care for members with chronic illnesses, comorbidities, and/or disabilities. Collaborate with RN or LSW, members, and the health care team to ensure cost‑effective and efficient utilization of health benefits. Make clinical decisions based on current member needs, disease processes, and clinical guidelines, without requiring nursing judgment. Support the overall case management process within the scope of licensure. Minimum Qualifications Current, unrestricted license as LSW, LCSW, LMHC, LMSW, or other non‑RN license allowed by state law. Stipulated education: Nursing Diploma, AS in Nursing, or related field. Minimum of 2 years’ experience in a service coordinator or similar role working with individuals with chronic illnesses, comorbidities, and/or disabilities. Ability to work independently in a field‑based environment. Preferred Qualifications MA/MS in Health or Nursing. State‑specified certification, if required. Experience traveling to work sites and other locations.
