Fairview Hospital / Cleveland Clinic
Role: Licensed Practical Nurse (LPN) providing home healthcare to pediatric and adult patients in their own homes.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Provide skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiological and psychosocial changes.
- Collect, analyze, and interpret health data, documenting diagnoses and potential issues.
Qualifications & Skills
- Current LPN/LVN license in the practicing state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
