RESPONSIBILITY TO MINISTRY

It is expected that all of the duties and responsibilities of this position will be performed in a manner that reflects the mission and ministry of Baptist Life Communities which is to administer the Spirit of Christ to the elderly and to families and in whatever ministries deemed to be consistent with and supportive of this mission in the provision of not-for-profit health care, other aging services and other services in a Christian environment based upon the examples and teaching of Christ.  It is expected that the person in this position seeks excellence in their own work, ever mindful of Baptist Life Communities ministry under the profound influence of the Gospel of Jesus Christ.

BASIC FUNCTION

The purpose of this position is to manage the admissions process by maintaining waiting lists for occupancy, interacting with the discharge planners at local acute facilities, responding to requests from community for information regarding placement at the campus, providing support for the facility interdisciplinary team to best provide appropriate placement within our system at the level and payer group needed to be of service to the residents and resident families that we serve.  This involves maintenance of support files and documentation, being knowledgeable of regulations regarding Medicare and Medicare and regulation in the state of Kentucky, interfacing with local and community persons in Human Services and other facilities to best serve our residents and resident families.

Administrator/Executive Director.

EDUCATION, QUALIFICATIONS AND CREDENTIALS

High school education required, 2-year degree in nursing, business or human services desired, 4-year degree in nursing, business or human services preferred .

JOB ROLE

• Receive direction from the Nursing Home Administrator on overall QAPI objectives for the organization.
• Be knowledgeable in data collection, data analysis methodology and performance improvement methods needed to support and lead performance improvement projects.
• Identify opportunities for improvement through analysis of data, observation of operations and consultation with leadership and staff.
• Collaborate with the quality committee and senior leaders to prioritize and develop QAPI efforts.
• Lead performance improvement projects and provide education and coaching in order to build needed skills in others to lead PIPs.
• Participate in multidisciplinary QAPI activities.

ESSENTIAL JOB FUNCTIONS

The employee must be able to perform each essential function effectively to be successful in this position:

1. Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.
2. Maintain occupancy at the building at the highest level possible, with the goal being 100% occupancy 95% of the time.
3. Provide assessment request and preliminary documentation to the Director of Nursing to do clinical assessment to determine appropriateness of placement.
4. Completes PASRR screen.
5. Assist families and residents through the complex process of admission to the facility, including but not limited to:
   • Physician designation
   • Power of Attorney designation
   • Estate recovery documentation
   • Community-based waiver
   • Advance directive—Family Decision Rights
   • Services available to resident
   • Service charges payable by resident
   • Medicare denial
   • Resident care conferences
   • Security valuables precautions
   • Personal clothing/belongings policy
   • Laundry policy
   • Access and visitation rights
   • Pharmacy service
   • Resident rights
   • Barber and beautician charges
   • Resident Trust Fund
   • Application for Medicare/Medicaid benefits
   • Rules and regulations of long-term care
   • Grievance procedure
   • Transfer and discharge policy
   • MR/MI screen
   • Privacy Act statement
   • Medicare Secondary Payer questionnaire
6. Maintains census data and works with Central Billing Office to ensure that billing is correct as to days and payer.
7. Provides admissions notice to various departments of Village Care Center, such as Dietary, Social Services, Nursing, etc.
8. Creates, monitors and maintains resident business files on-site regarding information on admission, etc.
9. Works with families, nursing staff and social services on discharge planning of residents when required.
10. Participates in planning, development and implementation of strategic changes and/or modifications to operations necessary to insure viability and appropriate operational issues are addressed.
11. Other tasks and duties as assigned by the Administrator/Executive Director.

KNOWLEDGE, ABILITIES AND SKILLS

Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to make presentations before top management, public groups and staff. Ability to define problems, collect date, establish facts and draw valid conclusions. Ability to read, analyze and interpret journals, financial reports and legal documents. Ability to apply mathematical concepts such as fractions, percentages, ratios and proportions to practical situations.
