Case Manager

Helping Hand

Helping Hand is seeking a dedicated Case Manager/QIDP to join our Case Management Team. You will provide person-centered support to help individuals pursue goals, exercise informed choice, and live independently in the least restrictive environment. You will coordinate services, advocate for individual needs, and collaborate with families, providers, and partners to ensure quality of care. On-site, 40 hours weekly with some evenings/weekends as needed. #J-18808-Ljbffr
