This role helps patients honor their goals and wishes, promotes dignity at end of life, and offers emotional, social, and resource-based support throughout the hospice journey. The Medical Social Worker collaborates closely with the interdisciplinary team to enhance comfort, quality of life, and overall patient and family well-being. Responsibilities include counseling, resource navigation, advocacy, and coordination with community agencies, along with maintaining timely documentation and meeting organizational productivity expectations. The ideal candidate demonstrates strong communication skills, empathy, professionalism, and the ability to adapt to the unique needs of patients, families, and the care environment.Responsibilities include:Honor patient wishes and support them in achieving their goalsHelp patients leave this world with dignityProvide counseling and support, including coping strategies, anticipatory grief, and long-term care planningCollaborate with the interdisciplinary team to ensure comfort and quality careConnect patients and families to community resources and referralsCommunicate with social and community agencies, especially for high-risk familiesMaintain documentation and productivity standards according to defined organizational expectationsParticipate in team meetings, trainings, and community eventsDemonstrate initiative, adaptability, cooperation, and dependabilityRespect diverse beliefs, choices, and lifestylesPerform additional duties as assignedRequirementsMaster of Social Work (MSW) degree from a graduate school accredited by the Council on Social Work EducationLicensed Clinical Social Worker (LCSW) preferredMinimum of one (1) year of clinical geriatric experience required; hospice experience preferredDemonstrated ability in casework and counselingStrong communication and counseling skillsDemonstrated skills in assisting individuals and families with problem-solving and navigating community resourcesAbility to efficiently operate a computer; prior Electronic Health Records (EHR) experience preferredValid California driver's license and a reliable, insured vehicleComfortable being mobile and traveling throughout the service areaOpen to an occasional flexible schedule, including evenings or weekendsSalary Description $37 - $54
