Hospice RN Case Manager (PRN Home Visits)

Grant Memorial Hospice, part of LHC Group within the Optum family, is seeking a Registered Nurse Case Manager to deliver and coordinate the Plan of Care for hospice patients during home visits within a 50 mile radius. This PRN position offers on-call evening/weekend hours and a supportive environment focused on compassionate care. Responsibilities include initial nursing evaluation within 48 hours, developing and implementing the care plan with the IDG, documenting progress, and guiding patients #J-18808-Ljbffr LeadingAge Virginia
