What You’ll Do: Key Responsibilities

As the Care Coordination Director, you’ll be at the heart of our operations, ensuring that care services are delivered with excellence. Your responsibilities will include: - Overseeing and managing all aspects of care coordination to ensure clients receive the highest quality support. - Leading and mentoring a team of care professionals, fostering collaboration and growth. - Developing and implementing care plans tailored to individual client needs. - Collaborating with internal and external stakeholders to optimize care delivery. - Monitoring and evaluating the effectiveness of care programs and making improvements as needed. - Ensuring compliance with all relevant regulations and standards.

What We’re Looking For: Skills and Qualifications

To succeed in this role, you’ll need: - At least 3 years of experience in care coordination, healthcare management, or a related field. - Strong leadership skills with the ability to inspire and guide a team. - Exceptional organizational and problem-solving abilities. - Excellent communication and interpersonal skills to work effectively with clients, families, and team members. - A deep understanding of care planning and coordination processes. - A commitment to upholding the values of compassion, integrity, and excellence.
