Pediatric ICU, hospice, and/or end-of-life care experience preferred. Pay Range: $38.20 - $57.30 per hour Where You Will Work: The Palliative Care Department is part of the Levine Children's Specialty Center, located at 1001 Blythe Blvd, Suite 200, Charlotte, NC 28203. The department partners closely with inpatient families to support children with complex medical needs. Job Summary: The Palliative Care Nurse Coordinator provides direct patient care and care coordination for pediatric patients and their families. Responsibilities include conducting bedside visits for symptom assessment and management, delivering family education on disease progression and goals of care, documenting all interactions in the EMR, and communicating clinical updates to the care team. The coordinator also triages and initiates palliative care consults, coordinates multidisciplinary goals‑of‑care meetings, collaborates with an interdisciplinary team to ensure timely, patient‑centered care, supports clinical workflow, educates staff on palliative care principles, coordinates with community partners, and facilitates continuity of care across inpatient and outpatient settings. We’re Looking For Graduate of an accredited or approved program in nursing Registered Nurse license issued by the state in which you practice At least 2 years of RN experience in an acute care setting Pediatric acute care experience Hospice or end-of-life care experience What You’ll Do Coordinate the clinical care of patients being evaluated for and/or already participating in the program, including research activities and education. Coordinate clinical care throughout the program utilizing pathways and developing tools to ensure high quality, cost‑effective continuity of care for all patients.
