Remote Case Manager - Home Care & Community Health (OH)

CareStar, Inc. is seeking a Case Manager to support individuals across Ohio's Lorain, Erie, Huron, Ashland, Cuyahoga, and Medina counties. This remote-friendly role involves assessing needs, developing person-centered care plans, and connecting clients with Medicaid and community services while coordinating across behavioral health, medical, and community-based systems. You will conduct eligibility evaluations, arrange home visits within a 2-hour travel radius, and advocate for participants #J-18808-Ljbffr CareStar, Inc.
