Responsibilities
- Screen, evaluate, re-evaluate, and treat patients assigned to PT services.
- Develop and carry out appropriate, policy-aligned treatment plans; implement evidence-based programs.
- Coordinate treatment activities and communicate clearly with the interdisciplinary care team and patient/family.
- Document evaluations, treatment goals, progress, and adjustments to plans of care.
- Educate patients and family members on the rationale for treatments and expected outcomes.
- Maintain professional standards of care and comply with agency policies and regulatory requirements.

Requirements
- BS in Physical Therapy.
- Licensed Physical Therapist in VT and NH, or hold an unencumbered compact license.
- Minimum of one year of experience as a PT in a home healthcare setting.
- Ability to work effectively in a home health/hospice environment.
- Current eligibility to practice in the listed locations and adherence to licensure requirements.
