Job Description

Job Function: 01. Profiling / Transcribing: Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status. Reviews and transcribes all medication orders including specialized orders based on approved protocols accurately and timely verifying therapy, correct medication, route, dose and frequency. Makes recommendations for alternate treatments when necessary. Records relevant information and drug waste in EHR.Job Function: 02. Dispensing Medications: Dispenses medications, prescriptions, miscellaneous items accurately and promptly. Processes requests via phone, window, fax, and computer in a professional and timely manner. Obtains medications when needed to provide optimal patient care using economical or alternative options and taking into consideration patient admission status (inpatient vs. outpatient). Legibly signs all paperwork and/or labels associated with these processes.Job Function: 03. Checking: Checks unit dose accurately making corrections to patient profiles if necessary. Checks medications before being placed in automated dispensing machines and floor stock to assure accurate fill. Checks IVs, batching, patient own medications, repackaging and extemporaneous preparations for accuracy. Checks crash cart trays, rapid response boxes, and miscellaneous emergency kits. Legibly signs all paperwork and/or labels associated with these processes.Job Function: 04. Sterile Preparations: Prepares, supervises and ensures aseptic compounding of IVs including antineoplastic agents correctly and in a timely manner promoting efficiency and cost effectiveness. Reviews levels of batched IV medications, batches, supplies to avoid waste, documents all required information, and assures batches are checked and put away promptly. Demonstrates a strong knowledge base of IV equipment, sterile technique, total parenteral nutrition, and safe handling of hazardous drug preparation and precautions. Legibly signs all paperwork and/or labels associated with these processes.Job Function: 05. Clinical: Participates in clinical initiatives. Initiates appropriate clinical interventions and clarifications with corresponding documentation. Demonstrates compliance with Pharmacy and Therapeutics Committee initiatives. Reviews pertinent information to monitor patients' therapies and makes appropriate recommendations eliminating duplicate, unwarranted, or inappropriate therapy. Converts non-formulary medication requests to appropriate formulary alternatives whenever possible.Job Function: 06. Drug Information: Provides drug information to physicians, nurses, other health care professionals, and patients/family members. Demonstrates and shares knowledge of various drug information resources and knows how to research and interpret data and information.Job Function: 07. Performance Improvement/Customer Satisfaction: Answers phones, window, and door promptly and in a courteous manner. Able to handle problems/issues independently. Participates in internal and external performance improvement and cost savings initiatives. Supports and actively participates in departmental and system initiatives. Seeks opportunities to work independently on projects, teams and committees. Is professional and helpful to patients, coworkers, and allied health professionals. Directs and monitors technicians assisting when necessary to maintain workflow.

Qualifications

Education/Experience:

• Graduation from an accredited School of Pharmacy with PharmD required
• Current active registration and licensure with the State of New Jersey Board of Pharmacy required
• Previous hospital pharmacy experience (2+ years) preferred.

About the Team

Atlantic Health System Pharmacy Departments service multiple outpatient areas, including several infusion centers, retail pharmacy services, and health clinics. At Morristown Medical Center and Newton Medical Center we have full-service retail pharmacies. They provide prescription medication services to patients and the local community in a convenient location on the first floor of the hospital, inside the Corner Store Gift Shop at Morristown and behind the coffee shop at Newton adjacent to the main entrance. A convenient service provided in our retail pharmacies is having your prescriptions you receive at discharge filled and ready for pick-up before you leave to go home. That means the discharge process will be a little easier for you and your family or friends since there is no need to stop at your retail pharmacy on the way home. The Pharmacy Department also works with physicians and nurses to ensure that medications used at Atlantic Health System are safe and effective. Pharmacists are integrated into your care teams to provide real time medication information and help you get the most benefit from your medications.
