Position Details Position: LVN Case Manager Schedule: Full-Time Annual Salary: $114,400 Location: Medical Hill Healthcare Center Key Responsibilities Coordinate and oversee resident care plans from admission through discharge. Collaborate with physicians, nursing staff, therapists, residents, and families to ensure continuity of care. Monitor clinical progress and identify opportunities to improve patient outcomes. Facilitate care conferences and interdisciplinary team meetings. Assist with discharge planning and transitions of care. Ensure compliance with federal, state, and facility regulations. Maintain accurate clinical documentation and case management records. Advocate for residents while promoting quality care and customer satisfaction. Qualifications Current California LVN license in good standing. Case management, skilled nursing, or long‑term care experience preferred. Strong assessment, communication, and organizational skills. Ability to manage multiple priorities in a fast‑paced healthcare environment. Knowledge of Medicare, Medi‑Cal, managed care, and discharge planning processes is a plus.
