Licensed Practical Nurse (LPN)

Full-Time & Part-Time | Lake Pointe Health & Rehabilitation | Conneaut, OH

We are currently seeking Licensed Practical Nurses (LPNs) to join our night shift team at our Skilled Nursing Facility in Conneaut, Ohio . We have 2 Full-Time and 1 Part-Time openings for compassionate, dependable nurses who are committed to high-quality resident care.

Position Details

Shift: Night Shift – 7:00 PM to 7:30 AM
Employment Type: 2 Full-Time | 1 Part-Time
Location: Conneaut, OH (serving Ashtabula County and surrounding areas)

LPN Responsibilities

Provide direct nursing care to residents in accordance with care plans
Administer medications and treatments as prescribed
Monitor residents’ condition and report changes to RN or physician
Accurately document care in electronic medical records
Collaborate with CNAs and nursing team to ensure resident safety
Maintain compliance with state and federal regulations

Qualifications

Active Ohio LPN license (required)
Experience in skilled nursing or long-term care preferred
Strong clinical and communication skills
Ability to work independently on night shift
Compassionate, reliable, and resident-focused
