Location: Homestead, FL (Field-Based) Schedule: Monday - Friday | 8:00 AM - 5:00 PM (Standard Business Hours) Compensation: $36.07 per hour + Full Benefits Travel: 75% Field-Based (Face-to-face member visits in Homestead and surrounding South Dade areas)

Role Overview

We are seeking a self-motivated, energetic, and highly organized Case Management Coordinator to join our South Dade team. This role is designed for a local professional who understands the unique needs of the Homestead community. You will act as a vital link between members and the healthcare system, balancing telephonic coordination with consistent face-to-face engagement.

You will manage a diverse caseload-ranging from supportive care to medically complex individuals-to evaluate medical needs, determine appropriate services, and facilitate overall wellness through proactive intervention.

Key Responsibilities

• Local Assessment & Planning: Conduct telephonic and face-to-face assessments to identify member health needs and social determinants of health within the Homestead area.

• Care Coordination: Develop and implement individualized courses of action, including coordinating with PCPs, skilled providers, and local community resources.

• Service Authorization: Manage prior authorizations and perform medication reviews to ensure members receive timely and appropriate care.

• Field Engagement: Travel extensively (75%) within the Homestead/South Dade region to meet members in their homes or community settings.

• Documentation: Maintain accurate, tech-savvy records of all interventions and care plans in a fast-paced environment.

Required Qualifications

• Bilingual Proficiency: Must be fully fluent in English and Spanish (verbal and written).

• Location Requirement: Candidates must be located in Homestead, FL or the immediate surrounding South Dade area.

• Transportation: Reliable transportation is required for daily field visits ( Mileage is reimbursed to and from member homes).

• Tech Literacy: Proficiency with digital platforms and case management software.

Pay Details: $36.07 per hour
