Pharmacist II Clinical Pharmacist

A direct hire opportunity is available for a Pharmacist II Clinical Pharmacist in Utica, NY. This role involves working in a decentralized clinical environment such as the emergency department or intensive care unit, providing critical pharmacy services that directly impact patient care and safety.

Responsibilities include:

• Deliver clinical pharmacy services in assigned hospital units, collaborating closely with healthcare teams to optimize medication therapy.
• Independently review and verify medication orders for appropriate dosing, routes, interactions, allergies, and indications.
• Participate in multidisciplinary rounds, offering expert recommendations on drug therapy.
• Conduct medication reconciliation, patient counseling, and provide drug information consultations.
• Monitor medication profiles to identify and resolve drug-related problems.
• Support and precept pharmacy residents, contributing to educational activities and program evaluations.
• Prepare sterile IV admixtures and compounded sterile products in line with USP standards.
• Train and oversee pharmacy technicians and staff in sterile compounding.
• Mentor pharmacy students and engage in clinical education efforts.
• Contribute to medication safety initiatives, quality improvement, and policy development.
• Ensure compliance with New York State and federal pharmacy regulations.

Qualifications:

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active New York State pharmacist license in good standing.
• 1 to 3 years of hospital pharmacy experience.
• Clinical expertise in hospital settings such as emergency departments or intensive care units.
• Completion of a PGY1 pharmacy residency.
• Board certification by the Board of Pharmacy Specialties (BPS) preferred.
• Experience in resident or student precepting and strong clinical pharmacotherapy knowledge.

This position reports to the Pharmacy Operations Manager, Clinical Pharmacy Manager, or designee. Schedule details will be determined.
