Licensed Practical Nurse (LPN) - Pediatric & Adult Home Health | Private Duty Nursing

Position Overview PRN, Part Time or Full Time – flexible hours with weekly pay. Join Team Select Home Care as a Licensed Practical Nurse (LPN) to provide one‑on‑one bedside care to pediatric and/or adult patients with complex medical needs in the comfort of their homes. This role offers a supportive team environment, structured training for new graduates, and opportunities to develop long‑term relationships with patients and families. Key Responsibilities Provide skilled nursing care under the direction of an RN. Administer medications and treatments based on individualized plans. Educate and support families in in‑home care techniques. Observe and report changes in patient condition and document visits using the EMR system. Assist the RN and physician with specialized procedures and preparation of equipment. Teach and counsel patients and families regarding nursing care needs and related problems at home. Assist patients in learning appropriate self‑care techniques. Follow and implement the nursing care plan for each patient as approved by the RN/physician. Participate in medical record audit, in‑service training, staff meetings, and patient care conferences. Perform on‑call duty, nights, weekends, and holidays as assigned. Maintain accurate records, comply with HIPAA, and complete documentation in a timely manner. Detailed Clinical Responsibilities Furnishes services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies. Prepares clinical and progress notes. Assists the physician, intermediate care provider and RN in performing specialized procedures. Prepares equipment and materials for treatments observing aseptic technique as required. Initiates preventive and rehabilitative nursing procedures as appropriate. Observes signs and symptoms, reports to the physician and RN nurse reactions to treatments. Evaluates with RN the effectiveness of the LPN's nursing service to the patient and family under the guidance of the RN. Participates in patient care conferences to discuss the involvement of other members of the health team. Assists in reviewing, evaluating, and updating the patient plan of care as appropriate. Administers medications and treatments as ordered; documents appropriately. Provides information regarding patient to other agencies or individuals involved in the patient’s care. Accepts, writes, and executes orders; orders are cosigned by RN where applicable. Ensures the patient/family is involved in implementing the plan of care. Makes decisions based on sound judgment and critical thinking. Maintains acceptable attendance status and is punctual. Demonstrates effective communication to patient/family, team members, and other health care professionals. Complies with all state, federal, and accreditation policies and procedures. Qualifications Graduate of an approved school of practical nursing and currently licensed in the state(s) in which they practice. Active LPN license. One year of nursing experience preferred. Active and valid CPR certification. IV certification required by state mandate, if applicable. Average computer skills and competency in media communications. Physical ability to perform described duties. Excellent verbal and written communication skills. Ability to exercise initiative and independent judgment. Passion for working with pediatric and/or adult patients in a home setting. Excellent communication and critical thinking skills.
