Position Summary & Mission This position provides direct clinical services to clients in the Felong Mental Health Court/FACT program with up to 75% of services occurring in the community. Provides counseling, comprehensive case management, and advocacy for FACT clients, with the goal of reducing recidivism into the Jail, reduce reliance on Emergency Rooms, and reduce use of both medical and psychiatric inpatient hospitals when appropriate. Assists clients with developing and using natural supports, with obtaining and maintaining stable living situations, obtaining necessary medical care and with the development of healthy and positive coping skills. Attends court with clients on a weekly basis and responds immediately to crisis situations with FACT clients as necessary. Responsible for rotating on-call phone shifts as assigned. The SUDP will serve as the primary point of contact for clients on their program caseload and is responsible for screenings, assessments, development of treatment plans and providing individual and group counseling. Additionally, the SUDP will complete all ASAM assessments for the entire FACT caseload, both in and out of custody. Ability to pass a jail background check is required. This position provides program information and facilitates referrals to outside resources to facilitate recovery and ensure client compliance with all legal requirements. Essential Functions / Competencies Provides effective clinical case management services to improve FACT clients’ ability to live successfully in the community. Performs on-going clinically appropriate assessments of consumers. Develops, with consumer voice, the goals to be addressed in individual treatment plans, tailored care plans, and crisis plans, in accordance with the appropriate model of care. Provides behavioral counseling, crisis and stabilization services to consumers. Maintains professional clinical boundaries with consumers. Provides wrap-around case management and referrals as clinically indicated. Provides advocacy with the court system, the jail, and other community supports as needed. Maintains clinical competency and resource knowledge required to meet changing consumer, community, and industry trends. Completes written reports for the court and attends scheduled court review hearings with clients regularly. Attends treatment planning meetings and daily team meetings. Assures confidentiality is always maintained. Provides direct SUD service to FACT program Complete SUD assessments for all FACT clients at the start of their program and as needed. Make appropriate referrals for treatment based on ASAM criteria and client’s benefits Serve as team point of contact for all SUD treatment providers (internal and external) Oversee UAs collected in the program, including communication with the lab and reporting results to the team. Complete Recovery Plans with individuals throughout the program, prior to graduation, and after any return to use. Provides individual support to FACT clients who may not meet ASAM criteria for enrollment into SUD services. Completes clinical documentation in a timely, accurate manner Develops individualized Treatment Plans with each client, including measurable goals and objectives for recovery and wellness, with a focus on reducing recidivism. Reviews and updates Treatment Plans with each client as frequently as needed, but at a minimum every 180 days. If a Crisis Plan is needed, formulates it with the client, addressing all required elements. Completes Discharge and Transfer Summaries in a timely manner, as clinically appropriate. Completes progress notes Current YTD progress note compliance percentage (will be calculated for PR). Progress notes routinely meet medical necessity requirements. Provides Secondary Therapist Functions Effectively Collects UAs regularly Accompanies and supports co-workers on outreaches. Provides effective crisis and stabilization services as needed. Develops and/or facilitates effective groups. Serves on and contributes to department or agency committees as needed. Attends all Program and Agency mandated trainings and meetings. Other duties as assigned. Completes administrative documentation Completes and submits Time Sheets, Time-Off Requests, Mileage Reimbursement forms, etc. accurately and on time. Provides all required annual documentation to Human Resources and completes all required Workday Learning training in a timely manner. Job Scope Clinicians work semi-independently managing a caseload of consumers in a highly competent and professional manner. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, clinical outcomes, and liability implications. Knowledge And Skills In depth understanding of the SUD population, ASAM criteria, modalities of treatment and services for adults and special populations. Ability to operationalize those treatment options within the managed care environment of the Pierce County MCO/ASO Skilled in a broad range of SUD treatment methods and techniques for working with diverse populations and carious client issues. Direct clinical experience in the assessment and treatment of a wide range of SUD and addiction needs A knowledge and understanding of mental disorders and basic familiarity with the DSM-V Knowledge of the major components of a mental status examination and competency in performing on-going assessments Ability to implement a continuum of care for consumers based on consumer need and level of functioning A basic knowledge of psychotropic medications with the ability to recognize side-effects Knowledge of community resources and the ability to develop appropriate links for consumers Well-developed skills in managing time, priorities, and paperwork; a sensitive perception of the needs of individuals with disabilities Keen understanding of the importance of clear, professional boundaries, and maintenance of such Possession of proven teamwork skills Excellent written and oral communication skills; and demonstration of good common sense Ability to work effectively in a team environment and to collaborate effectively with external partners is an absolute must Cultural competence General Functions / Competencies The following General Competency Expectations help create an environment which is physically and psychologically safe for both staff and clients: Customer Service - Demonstrates a positive attitude and efforts toward meeting the needs of internal and external customers. Mission focused – Demonstrates behaviors & practices that help to promote the values of MultiCare Behavioral Health Education And/or Experience The Substance Use Disorder Professional (SUDP) operates in a dual capacity as a therapist on the FACT team. In addition to an SUDP license, a BA or MA in a mental health or related field is required. Job Conditions Must be able and willing to drive a car both in cities and on highways; possess a good driving record; provide proof of auto insurance; be able to work both in and out of the office and go into consumers’ residences, which may require climbing stairs and walking on uneven terrain. Must be able to speak and hear at normal conversational levels. Must pass the background check in order to see program clients in the Pierce County Jail. Responsibilities You will perform Substance Use Disorder assessments in office-based, inpatient, or community settings as necessary; develop treatment plans, monitor patient progress, and conduct individual and group counseling sessions within the provisions of MultiCare’s policies and standards, as well as local, state, federal and other regulatory requirements You will respond to referrals from various locations in the community in the most effective, efficient and safe manner possible in order to perform Substance Use Disorder assessment and make treatment recommendations and care coordination You will provide care coordination and case management services, brief therapy, and motivational interviewing with a focus on rapid placement into the least restrictive appropriate level of care You will develop relationships with community resources and develop treatment and safety plans to assist individuals and their natural and professional supports in resolving chemically induced crises You will provide and connect patients and professionals with accurate program and agency resources You will complete all required documentation Requirements Bachelor's degree in related field required Current WA State Substance Use Disorder Professional certification Washington State Counselor Agency Affiliated Registered (ACCR) credential must be applied for within thirty (30) days of hire.
