What You'll Do

• Provide outpatient and inpatient physical therapy services
• Manage occasional SNF, home health, and school-based patient caseloads
• Document patient progress using Epic EMR system
• Collaborate with multidisciplinary teams to develop and implement treatment plans
• Educate patients and families on therapy exercises and safety
• Participate in weekend-on rotation (every 6–7 weekends) and rare holiday shifts

What You Need

• Minnesota Physical Therapist License (active)
• Basic Life Support (BLS) certification within one year of hire
• At least 1 year of relevant experience
• Strong knowledge of electronic medical records (EPIC preferred)
• Effective communication and teamwork skills
