We are seeking an experienced, motivated, and relationship-driven Community Outreach & Healthcare Marketing Specialist to expand our presence throughout Delaware County and surrounding communities .

This role is ideal for a healthcare marketing, business development, community outreach, or referral relations professional who knows how to build trusted relationships, develop referral partnerships, identify new opportunities, and drive growth .

? What You'll Do ? Community Outreach & Referral Development

• Develop and maintain strong referral relationships with hospitals, rehabilitation centers, skilled nursing facilities, physicians, discharge planners, case managers, social workers, hospices, assisted living communities, senior care organizations, and other healthcare professionals throughout Delaware County.

• Conduct regular in-person visits, presentations, and outreach activities with referral partners.

• Identify and develop new referral sources and strategic community partnerships .

• Educate healthcare professionals, community organizations, and families about At Peace Healthcare Agency's services.

• Represent the agency at health fairs, networking events, healthcare meetings, community events, and professional organizations .

• Increase awareness of At Peace Healthcare Agency and establish the organization as a trusted healthcare resource in the community.

? Healthcare Marketing & Business Development

• Develop and execute strategic outreach plans to increase referrals, admissions, and agency growth .

• Build and maintain a strong referral pipeline.

• Identify new business development opportunities within Delaware County and surrounding areas.

• Work toward established referral and business development goals.

• Track outreach activities, referral sources, and results using CRM and agency reporting systems.

• Provide leadership with feedback regarding market trends, community needs, referral opportunities, and competitive activity .

• Assist with marketing campaigns, promotional materials, presentations, and community education initiatives.

? Relationship Management

• Establish long-term relationships with referral partners based on trust, professionalism, responsiveness, and exceptional service .

• Maintain regular communication and follow-up with healthcare and community partners.

• Serve as a professional representative of At Peace Healthcare Agency throughout Delaware County.

• Strengthen existing partnerships while continuously developing new relationships.

• Work closely with agency leadership to increase community visibility and referral opportunities.

? Collaboration

• Collaborate with leadership, intake, operations, and clinical teams to support an efficient referral and admissions process.

• Communicate referral opportunities and client needs accurately and promptly.

• Help ensure referral partners receive timely follow-up and excellent service.

• Support community education initiatives that promote quality and compassionate healthcare services.

? What We're Looking For

We are looking for an experienced professional who can confidently represent a healthcare organization and develop strong referral relationships.

Required Qualifications

• Previous experience in healthcare marketing, healthcare business development, community liaison, home care sales, hospice marketing, physician relations, healthcare outreach, or a related field .

• Proven ability to develop and maintain professional relationships with healthcare and community referral sources.

• Excellent communication, presentation, networking, and relationship-building skills .

• Results-oriented and comfortable working toward measurable referral and growth goals.

• Self-motivated, proactive, and able to work independently.

• Professional demeanor with the ability to communicate effectively with physicians, healthcare executives, case managers, social workers, discharge planners, and other referral partners .

• Excellent organizational and time-management skills.

• Valid driver's license and reliable transportation for local travel.

⭐ Preferred Qualifications

• Experience in home health, home care, hospice, private duty, healthcare staffing, senior care, or related healthcare services .

• Existing relationships with hospitals, physicians, rehabilitation facilities, skilled nursing facilities, case managers, discharge planners, assisted living communities, or senior service organizations in Delaware County .

• Established healthcare or community referral network.

• Bachelor's degree in Marketing, Business Administration, Healthcare Administration, Communications, or a related field preferred but not required.

• Experience with CRM systems and Microsoft Office.
