About the Position

Job Title

Physical Therapist – Home Health

Schedule / Job Type

Contract | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across West Portland and surrounding areas
• Conduct patient evaluations and deliver individualized treatment plans on a 1:1 basis, visiting up to 6 patients per day
• Complete clinical documentation using EPIC Remote Client and Rover mobile application, including OASIS assessments
• Manage daily patient visit schedule, including travel between patient homes within an approximate 60-mile service radius
• Meet standard productivity expectations of 30 productivity points per day

Required Qualifications

• Active Oregon Physical Therapist license
• Minimum 1 year of skilled home health experience
• Current Basic Cardiac Life Support (BCLS) certification
• OASIS documentation experience required
• Reliable personal transportation required for daily patient visits
• COVID-19 primary vaccination series plus one booster required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for Home Health and Rover phone application
• Experience meeting productivity point expectations in a home health environment
• Comfort with variable daily scheduling and independent field-based work
