Resident Care Coordinator

Location: Phoenix, AZ
Job Type: Full-Time
Pay: $60K

About the Position

We are seeking a compassionate, organized, and dependable  Resident Care Coordinator (RCC) to join an assisted living community in Phoenix, AZ. The RCC plays a key role in coordinating individualized resident care, supporting caregiving staff, communicating with families and healthcare providers, and ensuring compliance with applicable regulations and community policies.

The ideal candidate is an experienced senior care professional who is passionate about helping residents maintain their health, safety, dignity, and quality of life.

Key Responsibilities

• Monitor residents' health status and update care plans as needed.
• Coordinate resident care services, including medication management and wellness support.
• Supervise, support, and provide direction to caregiving staff to promote high-quality care.
• Communicate regularly with residents, families, healthcare providers, and members of the care team.
• Ensure compliance with state regulations, licensing requirements, and company policies.
• Assist with resident admissions, transfers, and discharges.
• Maintain accurate, complete, and current resident records and care documentation.
• Respond appropriately to emergencies and ensure timely and accurate incident reporting.
• Participate in care conferences and interdisciplinary team meetings.
• Promote a safe, respectful, compassionate, and supportive environment for residents.
• Assist residents with mobility and other care needs as required.

Qualifications

• Experience in assisted living, senior care, or memory care.
• Strong knowledge of resident care planning and regulatory compliance.
• Excellent communication and interpersonal skills.
• Previous leadership or team-management experience.
• Strong organizational, problem-solving, and decision-making abilities.
• Ability to multitask and remain effective in a fast-paced care environment.
• Compassion, patience, empathy, and a genuine commitment to serving older adults and individuals with special needs.

Requirements

• Arizona Certified Caregiver License (Required)
• Valid Level One Fingerprint Clearance Card
• Current CPR/First Aid certification
• Must show proof of freedom from TB before client contact
• Provide and maintain current Food Handlers Certification
• Clean driving record and valid driver’s license (preferred for transport duties)
• Memory Care Certificate
• Clear the Department of Economic Security, Department of Aging and Adult Services Central Registry Background Check
• Ability to lift 50 lbs and assist with mobility as needed
• Strong work ethic, reliability, and compassion for individuals with special needs
