The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.DUTIES & RESPONSIBILITIESAssess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of careEvaluate home environmentAssess for muscle strength, mobility, gait, ROM—potential for rehabDirect physical therapy treatmentInstruct patients and families/caregivers in the use and care of therapeutic appliancesDetermine priority needs for physical therapyReport to physician patient’s reaction to treatment or changes in conditionInitiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patientInstruct them as to the goals of the physical therapy program for the patient by participating in case conferencesTrain patient in the use of prosthetic deviceIdentify patient and family/caregiver needs for other home health servicesPrepare and submit clinical and progress summaries based on the attainment of goalsParticipate in discharge planning for patientPrepare and submit a clinical progress summary based on the attainment of goalsProvide physical therapy consultation to home families/caregiversJOB REQUIREMENTSActive CA PT licenseMinimum of two (2) years of patient care experience; preferable in the home health settingPossess and maintain current CPR certification.Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderPAY RANGE: $50.00 - $60.00 per hourABOUT THE COMPANYBuena Vista Home Health is a subsidiary of the Pennant Group network with 300+ locations throughout the country.
