Certified CHW: Home-Visit Care & Community Support

BLEHEALTH

BLEHEALTH in Pomona, CA is hiring a full-time Community Health Worker for in-home and community outreach. You will engage clients, educate them on health resources, and help develop personalized care plans to improve outcomes. The role requires CHW certification, CPR, TB test, and a valid driver’s license.
