Comfort Keepers is looking for an organized, people-focused External Client Care Coordinator to help connect families with quality in-home care while supporting an outstanding client experience.

What You'll Do

• Build strong relationships with clients, families, caregivers, and referral partners
• Conduct in-home consultations to understand client needs and recommend services
• Coordinate care plans and caregiver schedules to ensure quality service
• Follow up with clients to maintain satisfaction and address concerns
• Collaborate with office staff to provide seamless care coordination
• Maintain accurate client records and required documentation
• Develop partnerships with healthcare providers and community organizations
• Support local networking events and community outreach initiatives
• Help identify opportunities to improve the client experience
