Job Description

The Wound Care Coordinator manages the facility's wound care program, ensuring high standards of care and practice in relation to wound, ostomy, and continence patient care. This role includes providing direct patient care, assisting staff nurses, and serving as a consultative resource for affiliated healthcare agencies. The coordinator collaborates to develop and implement patient care protocols, documentation tools, and wound care formulary while establishing care standards, competencies, policies, and procedures for effective wound care delivery. Furthermore, they act as the primary consultant for Wound Care Clinicians and participate in ongoing training, education, and clinical performance improvement initiatives.

Responsibilities

• Manage the facility wound care program and ensure compliance with national standards.
• Evaluate wounds upon admission and throughout treatment to determine care plans.
• Consult with physicians and assist in developing comprehensive wound management plans.
• Document wound assessments, care plans, and treatments accurately.
• Conduct root cause analysis for adverse events related to wounds and develop action plans.
• Participate in quality data reporting and performance improvement initiatives.
• Provide ongoing education and support to clinical staff regarding wound care products and protocols.

Required Experience / Certifications / Licensure

• Minimum of 3 years recent experience in Wound Care Coordination.
• ACLS and BLS certifications (must be through the American Heart Association).
• WOCN, CWS, or WCC certification.
• Current California RN license.
