Weekend Admission Nurse (RN) - Home Health

Part time Registered Nurse Case Manager Anticipated Territory: Sacramento County Schedule: Every Saturday and Sunday full days; may consider Saturday/Sunday plus 3 weekdays. Field Requirement: Must visit patients in their homes. Responsibilities Conduct Start of Care (SOC) Admissions - OASIS Conduct follow‑ups and evaluation visits as needed in the designated territory Make home visits to patients in the designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions and report changes to the clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided Benefits Concierge support for time‑consuming administrative tasks Flexible work schedules with autonomy Targeted territories; typically patient visits are within a 5‑15 mile radius No clocking in/out Opportunities for advancement (multiple RN tiers & clinical leadership) Low turnover/High morale environment Helpful software and communication tools Mileage/Vehicle reimbursement Company‑issued iPad Outstanding benefits package: 401(k) with matching, generous PTO, medical, dental, life insurance, etc. Competitive compensation with incentive bonus program Qualifications Current and unrestricted CA RN license Current BLS/CPR certification Valid CA driver’s license Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired High level of clinical knowledge in home health care preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team highly desired Case Management fundamentals highly desired Previous experience with IV Medication infusions highly preferred Proficiency in initiating intravenous (IV) therapy a plus Self‑directed, empathetic, critical thinker, outstanding interpersonal and leadership skills, organized 21st Century is an equal‑opportunity employer committed to fostering a diverse and inclusive workplace.
