Weekend Admission Nurse (RN) - Home Health

Part time Registered Nurse Case Manager Anticipated Territory Sacramento County Schedule Every Saturday and Sunday full days. Consider Saturday/Sunday plus 3 weekdays. What you’ll do: Conduct Start of Care (SOC) admissions – OASIS Conduct follow‑ups and evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to the Clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and administer IV medication infusions safely in accordance with established protocols Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided Benefits Flexible work schedules: create your own schedule and hours Targeted territories within a 5–15 mile radius No clock‑in/out (autonomy) Opportunity for advancement through RNs tier system and clinical leadership Low turnover, high morale Helpful software and communication tools Mileage/Vehicle reimbursement Company‑issued iPad Competitive compensation with incentive bonus program Outstanding benefits: 401(k) with matching, generous PTO, medical, dental, life insurance, etc. Qualifications Current and unrestricted California RN license Current BLS/CPR certification Valid California driver’s license Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired High level of clinical knowledge in home health care highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team highly desired Case management fundamentals highly desired Previous experience with IV medication infusions highly preferred Proficiency in initiating intravenous (IV) therapy is a plus Candidate Profile Self‑directed Empathetic Critical thinker Outstanding interpersonal skills Leadership skills Organized 21st Century Home Health Services is an equal opportunity employer, committed to fostering a diverse and inclusive workplace.
