About the Role


We are seeking an experienced Physical Therapist (PT) for a 13-week contract position in Waimea, Hawaii. The caseload is approximately 75% outpatient and 25% inpatient , including Acute Care, ICU, Med/Surg, Skilled Nursing Facility (SNF), and occasional additional hospital-based services.


The Physical Therapist must be comfortable practicing independently in a rural healthcare environment and managing a broad range of diagnoses. Responsibilities include evaluations, treatment, mobility progression, discharge planning, and coordination of care across inpatient and outpatient settings.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans for inpatient and outpatient patients.
• Provide therapeutic interventions to improve mobility, strength, balance, functional performance, and independence.
• Manage mobility progression and discharge planning for Acute Care, ICU, Med/Surg, and SNF patients.
• Treat a broad range of outpatient diagnoses while managing a mixed daily caseload of approximately 5-15 patients.
• Maintain accurate clinical documentation using Cerner and collaborate with interdisciplinary healthcare teams to coordinate patient care.

Requirements

• Active Hawaii Physical Therapist (PT) License Required.
• Minimum 2 Years of Physical Therapy Experience Required.
• Current BLS/BCLS Certification Required.
• Experience Across Inpatient, Acute Care, ICU, SNF, and Outpatient Physical Therapy Settings Required.
• Must Be Able to Work Independently and Be Available for Required Saturday Coverage and Flexible Scheduling.

The Physical Therapist will provide comprehensive rehabilitation services across inpatient and outpatient settings with a focus on mobility, function, safety, and independence.
