Work Environment

As a Physical Therapist in a home care environment, you will work onsite visiting patients in their homes in the Port Jervis area. Key Responsibilities

• Conduct comprehensive patient evaluations to assess mobility, strength, and functionality, utilizing evidence-based assessment tools.
• Develop and implement personalized treatment plans that incorporate manual therapy techniques, therapeutic exercises, and the use of assistive technology to facilitate recovery.
• Educate patients and their families about therapeutic exercises and activities to be performed between sessions to reinforce progress.
• Document all patient interactions, treatment plans, and progress notes in accordance with organizational protocols.
• Collaborate with a multidisciplinary team, including physicians, occupational therapists, and nursing staff, to ensure cohesive patient care and optimal recovery pathways.
• Manage a patient caseload of approximately seven clients per day, adjusting treatment strategies as necessary based on individual progress and response to therapy.

Patient Population / Client Focus

The patients you will serve primarily include individuals recovering from orthopedic surgeries, strokes, and other neurological impairments, and those dealing with chronic pain or movement disorders. You will have the opportunity to support a diverse range of patients, tailoring your approach to meet varying needs and recovery goals effectively.

Qualifications

• Active New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy (MSPT or DPT preferred).
• A minimum of two years of clinical experience in physical therapy, preferably in a home care or outpatient setting.

Preferred Experience

• Experience working with geriatric populations and managing complex rehabilitation cases.
• Familiarity with electronic health records (EHR) systems for documentation and patient management.
• Training in specialist areas such as sports rehabilitation, therapeutic modalities, or functional movement.

Compensation and Benefits
