This full-time onsite position focuses primarily on providing exceptional physical therapy services in a home care setting. Work Environment

This position requires physical therapists to work directly in patients' homes. Your work environment will be diverse and ever-changing, allowing you to provide personalized treatment plans within comfortable and familiar settings for patients. Throughout the week, you will primarily work from Monday to Friday, 9 AM to 5 PM, with flexibility to adjust based on patient needs. As part of our supportive team, you will have regular interactions with healthcare professionals, caregivers, and family members to ensure cohesive care delivery.

Key Responsibilities

• Conduct thorough assessments of each patient to evaluate their physical condition, including strength, mobility, and specific rehabilitation needs.
• Develop and implement customized rehabilitation protocols aimed at improving mobility, reducing pain, and facilitating recovery, while actively involving patients and their families in the process.
• Provide hands-on therapy interventions such as strength training, therapeutic exercises, gait training, and functional movement education.
• Educate patients and caregivers about safe mobility techniques and craft informative home exercise programs to encourage continued improvement between therapy sessions.
• Collaborate with a multidisciplinary team of healthcare providers, including occupational therapists, nurses, and case managers, to ensure comprehensive and coordinated care for patients.
• Document all therapy sessions meticulously, including treatment plans, progress notes, and outcomes, and ensure compliance with facility regulations and state laws.

Patient Population / Client Focus

As a Physical Therapist in the home care setting, you will serve a diverse patient population that includes geriatric clients recovering from surgery, individuals managing chronic conditions, and patients needing rehabilitation following injuries or debilitating health events. Your interventions will directly contribute to patients' functional independence, allowing them to safely navigate their daily lives with confidence.

Qualifications

• A graduate of an accredited Physical Therapy program (B.S., M.S., D.P.T., or equivalent certificate).
• Current licensure as a Physical Therapist in the State of New York.
• Current BLS HCP provider certification is required.
• Knowledge and adherence to the Physical Therapy Practice Act in New York and APTA Code of Ethics.

Preferred Experience

• A minimum of two years of clinical experience in a home care, skilled nursing, or outpatient therapy setting is preferred.
• Experience working with individuals in acute and chronic phases of physical rehabilitation.
• Proven ability to effectively manage a patient caseload while maintaining quality care standards.

Compensation and Benefits
