MDS Coordinator (Licensed Nurse) Tradewater Health & Rehabilitation part of Concord Health Systems in Dawson Springs KY Tradewater Health & Rehabilitation is a small, family‑owned nursing home committed to meaningful change, higher quality standards, and a renewed focus on honoring the fact that our building is the home of our residents. We are seeking an experienced MDS Coordinator who is a licensed nurse (RN or LPN) and is passionate about accuracy, accountability, and resident‑centered care. This role is essential to ensuring that every resident receives the quality of care they deserve — and that our documentation reflects the respect we have for their home.
