Key Responsibilities\n

As an integral member of our clinical team, your responsibilities will include:

• Conducting thorough clinical assessments of patients to identify emotional, mental, and psychosocial needs.

• Developing personalized treatment plans that incorporate evidence-based therapeutic interventions tailored to each individual's situation.

• Utilizing diverse therapeutic modalities such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused therapy to support clients in coping with grief, anxiety, and other mental health challenges.

• Maintaining accurate and confidential clinical documentation in compliance with HIPAA regulations and organizational standards.

• Collaborating with multidisciplinary teams through regular case consultations and team meetings to optimize treatment approaches and enhance patient care.

• Educating patients and their families about mental health issues, care options, and community resources to empower them in their journey.

• Remaining up-to-date with advancements in the mental health field to continually refine your clinical practice.

Patient Population / Client Focus\n

This position focuses on providing care to patients within a hospice setting, often dealing with complex psychological and emotional needs related to end-of-life issues. Your role will involve working closely with terminally ill patients and their families, providing essential support to facilitate coping and acceptance during a profoundly challenging period.

Qualifications\n

• Master's degree or higher in Social Work or a related field from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.

• Proven experience in delivering mental health services, particularly in hospice, palliative care, or with terminally ill populations is highly preferred.

• Exceptional interpersonal and communication skills, facilitating rapport-building with both patients and colleagues.

• Ability to work autonomously as well as collaboratively within a multidisciplinary team.

Compensation and Benefits\n
