Key Responsibilities\n

• Conduct thorough ongoing assessments to accurately identify and address the needs of each patient.

• Facilitate clear communication between supervisors, direct care staff, families, guardians, and other stakeholders regarding patient progress and treatment plans.

• Develop and provide a provisional schedule of appointments, home visits, and monitoring sessions to supervisors, ensuring structured support for clients.

• Organize home visits to gather comprehensive insights through observations, chart reviews, and staff interviews.
\n
• Implement the organizational program plan effectively, ensuring that all therapeutic interventions align with individual patient goals.

• Actively participate in Interdisciplinary Team Meetings to collaborate on complex cases and share insights on patient care.

• Maintain detailed monthly and quarterly progress notes to document client advancements and treatment efficacy.
\n
• Coordinate training programs to elevate staff competencies in program delivery and documentation procedures.

• Provide ongoing support through follow-up interviews and home visits, ensuring that patient needs are continually met.

• Maintain accurate and accessible patient records to facilitate information sharing with relevant parties.
