Remote Care Coordinator for Senior Health and Social Needs
VisionSPRR
UnitedHealthcare is seeking a Community Well Care Coordinator to support older adult members' medical and socioeconomic needs through effective care management. This position involves assessing clients' needs, developing individualized care plans, and collaborating with internal teams and community resources. Ideal candidates will have a Bachelor's degree in a relevant field and experience with older adults. The role is fully remote with a schedule of Monday to Friday. #J-18808-Ljbffr
