Senior Population Health Manager - Care Coordination

CaroMont Health seeks a Population Health Manager to oversee the assessment, planning, implementation and evaluation of health services across the continuum of care for a defined population. The role will coordinate with providers, multidisciplinary teams and community partners to improve outcomes and reduce readmissions. The ideal candidate holds an RN license in NC, a BSN or higher, and 5+ years in acute care with 2+ years in care coordination or post-acute care. #J-18808-Ljbffr CaroMont Health
