Job Summary The Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a Physical Therapist. The PTA educates patients and their families, ensures patient safety, completes accurate documentation, and remains compliant with all legal rules and regulations. Job Details Agency: Baptist Health Homecare – Lexington Schedule: Monday – Friday, 8:00am – 5:00pm; no weekends or on‑call requirements. Territory: Lexington, Paris, Salvisa, Lawrenceburg, Frankfort, Sadieville, Versailles, Midway Sign‑On Bonus: $5,000 Benefits Competitive base hourly rate with an uncapped bonus structure Mileage reimbursement Flexible schedule Medical, dental, and vision insurance (participants select what works best) Paid time off 401(k) participation Company‑paid life insurance Robust Employee Assistance Program Responsibilities Provides skilled services in a patient’s home as developed by the Physical Therapist’s plan of care. Observes, records, and reports patient reactions, changes, and progress. Performs physical therapy procedures and diagnostic tests as medically prescribed. Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, and water. Assesses functional problems and provides intervention; works toward goals to improve patient functional status. Communicates effectively with physicians, therapists, and coworkers. Participates in case conferencing to coordinate care. Recommends or evaluates prosthetic/orthotic devices, making minor adjustments or recommending modifications. Educates staff on specific modalities when appropriate. Educates patients and families on therapy techniques, safety measures, and supportive activities. Instructs patients and families on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other devices. Fosters independence in patients and families to prevent dependence on the agency. Completes all clinical documentation per agency protocol and Medicare/Federal guidelines. Completes chart audits and communicates scheduler changes. Participates in on‑call requirements, case conferences, in‑service trainings, mandatory agency meetings, and orientation/training of new employees. Drives to patient’s primary location per scheduled visit. Documents all aspects of visits within 24hours (discharge, evaluation, etc.) and within 48hours for start‑of‑care visits. Follows plan of care within the licensed PTA scope of practice as defined by the state. Qualifications & Attributes Physical Therapist Assistant with a current license in the state of employment. Minimum one year of PTA experience in an acute setting; home‑care experience desired. Effective communication and positive impression with patients, families, physicians, and coworkers. Knowledgeable of therapy best practices and able to make appropriate therapeutic judgments. Detail‑oriented, observant, and disciplined with the ability to prioritize and be timely. Valid driver’s license and auto insurance with your name as a listed driver. Health Qualifications Must meet all physical position requirements. Evidence of a current negative tuberculin skin test or equivalent at orientation; annual testing required thereafter.
