Key Responsibilities\n

• Provide comprehensive social work services to clients in hospice care, ensuring they receive the necessary emotional, psychological, and social support.

• Conduct thorough assessments of clients' needs, strengths, and personal circumstances to develop tailored treatment plans that address both immediate and long-term concerns.

• Advocate for clients, ensuring their rights are respected and that they have access to the resources needed for end-of-life care.

• Collaborate closely with interdisciplinary teams, including physicians, nurses, and other specialists, to coordinate comprehensive care and optimize patient outcomes.

• Manage case documentation and maintain accurate records in compliance with regulatory standards and organizational policies.

• Provide crisis intervention and support, using evidence-based practices to help clients navigate complex emotional challenges.

Patient Population / Client Focus\n

In this role, you will work primarily with clients in hospice care—individuals with terminal illnesses and their families. You will be instrumental in helping them cope with grief, loss, and the emotional turmoil associated with end-of-life decisions. Understanding the unique needs and dynamics of this client population will be critical as you deliver compassionate, culturally sensitive support tailored to each individual’s wishes and family contexts.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited university.

• Possession of a valid Licensed Clinical Social Worker (LCSW) license in Oregon.

Compensation and Benefits\n
