Work Environment

As an onsite Physical Therapist, you will work within the home environments of our clients, creating a safe and supportive atmosphere for rehabilitation. The role is primarily in the field, allowing for flexibility and varied daily experiences as you engage with different patients and adapt your care to each unique home setting. Your work schedule will primarily run from Monday to Friday, evenings and weekends as required for patient care.

Key Responsibilities

• Implement effective physical therapy interventions aimed at preventing disabilities, alleviating pain, and enhancing muscle function in accordance with physician-prescribed treatment plans.
• Supervise Physical Therapy Assistants to ensure precise execution of care plans and effective delivery of therapy services.
• Conduct thorough assessments of clients' functional levels using objective diagnostic tests, providing a comprehensive evaluation of their muscle, nerve, joint, and overall functional abilities.
• Evaluate clients' home environments, making recommendations that promote independence during rehabilitation.
• Deliver tailored physical therapy sessions in clients' homes, utilizing innovative techniques and evidence-based practices.
• Administer therapeutic exercises both manually and through guided instructions, facilitating crucial functional activities geared towards successful rehabilitation outcomes.
• Utilize modalities such as traction and massage while integrating modern physical agents to enhance treatment efficacy.
• Evaluate, fit, and adjust prosthetic and orthotic devices, collaborating effectively with physicians on necessary modifications.
• Participate in the creation and ongoing revision of comprehensive, personalized care plans based on continuous client assessments.
• Observe and meticulously document client progress and changes in condition to inform adjustments to care plans and report findings to the care team.
• Engage actively in interdisciplinary team meetings to discuss client treatment and progression, fostering a collaborative approach to patient care.
• Educate clients and families on therapy protocols and device usage, ensuring they understand and feel empowered in their care journey.
• Support and mentor Physical Therapy Assistants throughout their service delivery, ensuring high-quality care is consistently provided.
• Conduct on-site functional assessments, modifying care plans as necessary to adapt to changes in client needs or community resource availability.

Patient Population / Client Focus

Our clients range from those needing orthopedic rehabilitation following surgeries or injuries, to individuals seeking to manage chronic conditions affecting their mobility. You will work closely with patients of all ages, implementing specialized interventions that are adapted to their unique needs, abilities, and therapy goals.

Qualifications

• A Doctor of Physical Therapy (DPT) degree is preferred, but a Bachelor's degree with a license and relevant experience may be acceptable.
• At least one year of experience as a physical therapist, particularly in acute care or home care settings.
• A current, valid PT licensure in the state of Virginia.
• Hold a valid CPR certification and maintain all necessary credentials.
• A valid driver's license and automobile insurance are required.
• Exceptional communication and interpersonal skills, with a strong focus on patient/family engagement.
• Strong work ethic, a commitment to clinical excellence, and the ability to collaborate effectively in a team-oriented environment.
• Deep understanding of healthcare regulations and the commitment to delivering compassionate care.

Preferred Experience

• Demonstrated leadership qualities, capable of making informed decisions and advocating for patient needs.
• Experience with advanced therapy modalities and techniques conducive to home care settings.
• Previous exposure to developing care plans and modifying them based on patient progress.
• A proactive approach to mentoring and supporting the growth of junior staff, particularly Physical Therapy Assistants.

Compensation and Benefits
