POSITION SUMMARY

The Team Lead Case Manager?Willow Landing position is responsible for organizing, monitoring, assisting, and ensuring that all delivered services by the case management team meet the needs of the program. The team lead case manager acts as a liaison between case managers and the Willow Landing Program Director/Operations Manager.

The team lead is responsible for supervision of case managers while personally handling complex resident cases to ensure that residents receive quality, comprehensive case management services in accordance with ESSHI program objectives and regulatory requirements. Core duties include supervision of case managers, resident case management, information and referral, intake, record keeping and serving as an escalation point for crisis management. Some transportation and travel are required to meet the needs of the program participants and may vary depending upon the program. Hours will be determined based upon program staffing needs. Position is a 32 hour/week full-time position with benefits.

ESSENTIAL RESPONSIBILITIES

• Onboard and train new hires, conduct performance reviews, track performance, and ensure compliance with regulatory standards and operational practices
• Monitor daily workloads, assign cases, and maintain a reduced or specialized caseload
• Mediate difficult resident disputes, handle high risk escalations, and coordinate with community partners and service providers
• Maintains case records, data entry, dashboard, and completes necessary paperwork
• Monitor and assist engagement and connection to community services, linkages to education and career development opportunities, and life skills
• Attend regularly scheduled agency-wide meetings, community meetings and other meetings as assigned
• Oversee initial assessments to determine program eligibility
• Provide on-going case management support for residents including budgeting and other life skill assistance that promotes independent living
• Accompany residents to appointments to aid residents as appropriate
• Conduct meetings with case managers and residents to evaluate progress on individual and program goals
• Responsible for ensuring confidentiality of client files
• Responsible for ensuring data entry of clients in all applicable databases
• Oversee weekly case reviews/round ups including information gathered from others involved with residents
• Provide crisis intervention and deescalation support to residents and document issues as required
• Prepare quarterly reports on numbers served, status and successful outcomes. Provide additional reports as required
• Responsible for timely return of phone follow up with clients and providers
• Ensure the completion of housing unit inspections on a regular basis to help support a positive living environment Perform initial and annual inspections using HUD guidelines & Housing Quality Standards Inspection Form
• Participate in staff meetings, individual supervision meetings, training, and events
• Some flexibility to work hours as business necessitates and to best serve the residents of the program
• Performs other duties as assigned

• Maintain a safe work environment in a clean, organized, and safe manner. Conduct daily inspections of work areas to look for unsafe conditions
• Actively participate in health and safety practices and accident prevention
• Ensure all accidents are reported to supervisors and Human Resources, thoroughly investigated, and properly documented. Investigate near-miss incidents to determine root cause to promote accident prevention
• Use all required safety devices and personal protective equipment

Salary: $27-$30/ hour

REQUIRED QUALIFICATIONS

Education:

Minimum Qualifications

• High school diploma or equivalent with required experience

Preferred Qualifications

• Associate?s degree or higher in human services related field (social work, psychology, or mental health counseling)
• CASAC, or other relevant certifications highly recommended

Experience:

• 4 years of experience working in the human services field and/or Case Management Experience
• Experience with low income chronically homeless, youth, substance use, serious mental illness, DV survivors, veterans, and/or HIV/Aids populations needed
• Must have knowledge of entitlement benefits programs and resources available for vulnerable populations

Skills:

• Must have good communication and interpersonal skills and the ability to work with

diverse groups

• Computer literacy and database experience in excel and access helpful
• Must have current driver?s license and reliable vehicle for travel
