Overview Physical Therapist - Skilled Nursing Facility, Moses Lake, Washington. Start date 08/31/2026 for 19 weeks. Weekly pay range $2,384-$2,486 per week. Guaranteed hours 36 per week. Shift: 5x8 days. This travel temporary assignment is in a long-term care setting with a split placement to support a critical need. Key Highlights - Weekly pay: $2,384-$2,486 per week - Guaranteed hours: 36 per week - Contract duration: 19 weeks - Shift: 5x8 days - Start date: 08/31/2026 - Work setting: Long-term care / skilled nursing facility Responsibilities - Perform comprehensive PT evaluations to determine rehabilitation needs and create individualized treatment plans for residents in a skilled nursing setting. - Deliver physical therapy interventions (strengthening, mobility, gait training, balance, ROM) and adjust plans based on progress and goals. - Document evaluation findings, treatment plans, daily progress notes, and outcomes in resident records following policy and coding guidelines. - Monitor safety, prevent falls, and manage assistive devices, equipment, and adaptive strategies as needed. - Collaborate with nursing, medicine, therapy colleagues, and families to coordinate care, discharge planning, and transitions of care. Requirements - Active Physical Therapist license eligible for practice in Washington state or already licensed. - Doctor of Physical Therapy (DPT) degree or equivalent required. - Current Basic Life Support (BLS) certification. - 1+ year of physical therapy experience in a skilled nursing facility or similar long-term care setting preferred. - Ability to work effectively in a SNF environment with a focus on functional outcomes and resident safety. - Strong communication, documentation, and collaboration skills; comfortable with a temporary, split-placement assignment.
