Job Description
Registered Nurse (RN) - Home Health

We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health team. This position requires a compassionate professional who can provide high-quality care to patients in their homes.

Job Responsibilities

Conduct patient assessments and perform comprehensive evaluations of home health needs.
Develop individualized care plans based on patient assessments.
Administer medications, treatments, and therapies as prescribed.
Coordinate care with other health professionals to ensure optimal patient outcomes.
Documentation of patient care and progress accurately and promptly.
Provide education and support to patients and their families regarding health management.
Participate in quality improvement initiatives within the home health program.
Manage and document patient care in compliance with CMS/OASIS regulations.

Schedule

Day shifts from 8:00 AM to 4:30 PM, Monday through Friday.
Weekend on-call coverage required, averaging one weekend per month.
Overnight shifts may be required, typically 1-2 days per week from 4:30 PM to 8:00 AM.
Rotating holiday coverage required.

Qualifications

Minimum of 2 years of experience in a CMS/OASIS Home Health program is required.
5 years of experience in home health is preferred.
Current and active nursing license.
Reliable transportation is a must, with mileage reimbursement provided.
