Key Responsibilities\n

• Conduct thorough psychosocial assessments for individuals, families, and groups to form an effective, personalized treatment plan.

• Perform comprehensive initial patient assessments and triage care based on individual needs to ensure timely and effective interventions.

• Collaborate with interdisciplinary teams, participating in regular meetings and documentation to track progress and adjust care plans.

• Utilize your expertise in identifying the psychological, social, educational, and safety needs of diverse patient populations.

• Manage a manageable caseload by providing regular case management services, ensuring collaborative care with community providers and professionals.

• Deliver evidence-based therapeutic interventions, including CBT, to address the unique challenges faced by clients.

• Maintain accurate and timely documentation in compliance with departmental requirements and ethical standards.

Patient Population / Client Focus\n

Your role will include working with a varied demographic, including children, adolescents, and adults facing mental health challenges such as anxiety, depression, trauma-related disorders, and relationship issues. Qualifications\n

• Candidates must possess an active license as a Licensed Professional Counselor (LCPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW) in Illinois.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology is essential.

• Willingness to commit to working between 10 and 40 hours per week, aligning with patient needs and clinic capacity.

• A minimum of 2 years of clinical experience is required, ensuring a solid foundation in therapeutic practices.
